NURSING LICENSE?

Follow these steps to get a New Hampshire RN license:
Note: To be eligible for a New Hampshire nursing license by endorsement, you must have worked as a nurse for at least 400 hours in the last four years AND finished 30 education contact hours within the past two years OR passed the National Council Licensing Examination (NCLEX) within two years of applying for this licensure.
- Complete the application.
- Complete Declaration of Primary State of Residency form and attach a copy of your driver’s license.
- Pay the $120 fee.
- Send application, residency form and fee to the New Hampshire Board of Nursing Office:

- Verify licensures in other states through Nursys for Nursys-participating jurisdictions or by using the form. Your original state board needs to mail the verification to the New Hampshire Board of Nursing.
- Complete a background check using this form. Complete the information section with signature, indicate “ORI” code: NHNSP0800 STATE POLICE CONCORD, NH and “Reason for” would be: BOARD OF NURSING NH RSA 326-B:15. Leave “EMPLOYER AND ADDRESS” blank. The card should have your inked fingerprints or live scan images on it.
- You can obtain your fingerprints from whichever agency you generally use.
- Include the NH State Police processing fee of $47 (make checks payable to State of NH, Criminal Records).
- Mail completed fingerprint card, notarized Criminal Background Record Release Authorization form and fee to:
